Making Appointments and What To Bring


SCHEDULING FOR AN APPOINTMENT:
Please call our nearest branch OR log in to Patient Portal.
We are pleased to tell you that our office is located in an area easily accessible by car or bus. We also have ample parking space. Should you need further help with directions, please call us at above number as soon as possible.

Please plan to arrive at least 15 minutes prior to your scheduled appointment.

​ If you need to cancel, please call us at least 24 hours before your appointment to avoid a $60 no show fee. In the New Patient Packet (below) you will find important documents about our practice. Please review the documents carefully, and complete the demographic information, Consent, and Patient past, social, and family history forms.

In order for us to expedite your registration process, we request that you bring the following items with you:

Patient Registration Form, completely filled-out and signed
Medical History Form, completely filled-out and signed
Consent Form, completely filled-out and signed
• A list of all your current medications
A Valid insurance card(s), must be valid
Photo ID, preferably state issued
Co-pay, if it applies to your insurance

Please be aware that we will have to ask you to reschedule if you fail to bring the above items with you. Please do not hesitate to call our office if you have any questions.

PRESCRIPTION REFILLS


We require 24 hours notice for any prescription refills.

Please call during regular office hours as the on-call physician will not fill maintenance medications. We strive to have all refills called in by the end of the day. However, medications requiring physician verification may take extra time.

​When calling please have the name, strength, and directions for each medication as well as the prescription number and the pharmacy phone number.

​ Failure to provide this information may cause additional delays.

BMA accepts all major insurance carriers including but not limited to:

  • CareFirst BCBS
  • US Healthcare
  • Cigna
  • Coventry

  • Amerihealth
  • Medicare
Be prepared to provide all insurance information, including identification cards, for each visit.
* If you are covered by an HMO or other managed care product, you must contact your insurance carrier to make sure that Brandywine Medical Associates is listed as your Primary Care Provider. Failure to do so could result in you being responsible for the cost of your visit or rescheduling your appointment.

All co-pays are due at the time of visit.
